Reveal strengths of all children and families through play, family focus, and team collaboration by providing therapy services to children efficiently, ethically, and with the child's best interest in mind to achieve the individual's outcomes. Initiate, develop and carry out a treatment plan by completing a full evaluation and follow up documentation including; plan of care, progress report, and discharge in collaboration with the team of therapists and family. 
 

Job Tasks:

Treatment of diagnoses and disorders related to your PT therapy field in addition to:

​

  • Contact physicians as needed regarding patient care.

  • Refer in clinic for other disciplines that may be appropriate for additional services.
  • Ensure patients belong in our clinic through continual screening and progress updates.

  • Maintaining goals and progress each daily visit.

  • Patient and family tour and review of no show/cancel policy upon initial visit.

  • Patient management including evaluation, establishing a plan of care, being fully prepared for each daily visit, discharge and progress reporting.

  • Able to correctly identify diagnosis and prognosis for the plan of care.

  • Build a strong rapport with patients and families to promote collaboration and continuity of patient care evident by frequent arrival and progress of child.

  • Initiate discussion regarding plan of care and parent follow-through.

  • Patient contact and conflict resolution for frequent no show and cancellations.

  • Applied skilled intervention that results in patient goals being met with high patient satisfaction.

  • Actively participates in maintaining clinical key performance indicators and uses them to improve performance

  • Provide home programming at each visit to ensure family has follow through plan for home.

  • Completing visual schedules and therapy boards.

  • Supervise aides, assistants, and techs.

  • Maintains patient safety through screening, monitor vital signs and changes in patient and holds or adjusts interventions accordingly.
